How it Works

• You may have been advised to request a doctor's letter for the purposes of travel, employment, visa applications, housing etc. Often, you will need a medical examination for this to be appropriately completed. Sometimes, your doctor may be able to complete the letter by referring to your medical notes.
• Please fill out the form below giving as much information as possible. This service may incur a fee.
• Please allow 7 days before the letter will be ready for collection.
